Assignment Corrections

Complete all parts of the Assignment Correction Request Form.

Accurately correct ALL missed items on a sheet of notebook paper that is set up as described.  Click here for an example.

Turn in the fully completed Assignment Correction Form within 1 week of the date the assignment is sent home.

If all required parts are received by the date due, partial credit will be reflected on the original grade based on the student reflection and accuracy of the corrections made.

* Some “assessments” may not qualify for a retake.  For example, fluency checks will cover content on which students have already been taught and assessed.  For these checks, students are given a fluency review sheet that specifically covers the skills assessed - sometimes using the exact same assessment items on the check.

Students who score a failing grade on most assessments* can complete a test correction to receive partial credit.  All required steps must be completed as described below and on the form to be eligible for partial credit.
